Forgotten Victims Survey

This survey is designed to understand your experience as a "forgotten victim" as a person who accidentally caused serious injury or killed another person through a motor vehicle accident. The accident was not due to alcohol, drugs, road rage or racing. Your responses will help our organization decide how to set priorities and design new programs to serve our clients. If you are an adult forgotten victim, at least 18 years old, we invite you to participate in this survey. It takes about 10 minutes to complete.
